Ian Holloway’s side drew 2-2 with Birmingham at St Andrew’s to qualify 3-2 on aggregate and secure a clash against the Hammers at Wembley on May 19.
It means one of the three clubs relegated from the Premier League in 2011 will return at the first attempt, with Birmingham the other team that suffered the drop.
Goals from Stephen Dobbie and Matt Phillips put Blackpool in control of the tie at 3-0 up, but Nikola Zigic and Curtis Davies replied to set up a nervy finish.
The Tangerines won the play-off final two years ago by beating Cardiff and manager Holloway hopes his side can continue their fine form against West Ham this season, having won the two meetings 4-0 and 4-1.
“We’re going to Wembley again and it’s just magnificent,” said Holloway. “Words fail me. I’m so proud.
“We got ourselves into a brilliant situation but what a fantastic fightback from Birmingham. If they had equalised I couldn’t see us coming back.
“Once they got a goal it changed everything. It got the crowd on their side and we forgot to play.
“At the end of the day it was all about finishing the game with one more goal than them and I’m very proud of my boys for coming through.”
